Mindfulness Meditation
The most studied psychological intervention in oncology. Not a luxury β a clinical tool.
What it is
Mindfulness meditation involves directing attention to present-moment experience β breath, body sensations, sounds β without judgment or the attempt to change what is noticed. It trains the capacity to observe thoughts and feelings without being controlled by them.
Why it matters
Cancer diagnosis generates a torrent of fear, anticipatory grief, and rumination. This is normal β and also physiologically costly. Chronic psychological distress elevates cortisol, suppresses NK cell activity, impairs sleep, and degrades quality of life in ways that directly affect treatment outcomes. Mindfulness-Based Stress Reduction (MBSR) is the most evidence-backed psychological intervention in oncology, with effects on anxiety, depression, fatigue, and quality of life that rival pharmacological treatment in many studies. The goal is not to feel fine about a terrible situation β it is to reduce the secondary suffering that comes from fighting the reality of what is happening.
The evidence
A 2016 Lancet Oncology RCT found MBSR reduced depression symptoms in cancer survivors at 12 months. A Cochrane review of 21 trials found mindfulness-based interventions significantly reduced anxiety, depression, and fatigue in cancer patients. Meta-analyses consistently show effect sizes of 0.3β0.6 β comparable to antidepressant medication β for psychological distress outcomes.

How to practice
Start with 10 minutes daily. Sit comfortably. Close your eyes or soften your gaze. Bring attention to the breath β specifically the physical sensation at the nostrils, chest, or belly. When the mind wanders (it will, constantly), notice it without self-criticism and return. That noticing-and-returning is the practice β it is not a failure, it is the exercise. Build gradually to 20β30 minutes. Use apps like Waking Up, Headspace, or Insight Timer if you need structure. MBSR programmes (8-week group courses) are available through hospitals and online.

Notes
Mindfulness can surface difficult emotions β this is usually appropriate and part of the process. If you are experiencing acute trauma, intrusive thoughts, or dissociation, seek a trauma-sensitive instructor rather than practising alone. It is not a substitute for psychological support if you are experiencing clinical depression or acute suicidal ideation.
