Direct Bilirubin (Conjugated)
Elevated when the liver or bile ducts are the primary problem.
What it is
Direct (conjugated) bilirubin is the water-soluble form processed by the liver. When this fraction is disproportionately elevated, it points to intra- or extra-hepatic bile duct obstruction or hepatocellular damage.
Why it matters
Distinguishing direct vs. indirect bilirubin elevation helps localise the cause. Direct elevation → liver or bile duct issue. Indirect elevation → pre-hepatic (e.g., haemolysis). This distinction guides the next clinical steps.
