Milk Thistle (Silymarin)

The most evidence-based liver-protective supplement — critical during chemotherapy.

What it is

Silymarin is the active complex of flavonolignans extracted from milk thistle (Silybum marianum). The key active compound is silybin (silibinin). Milk thistle is one of the most comprehensively studied hepatoprotective agents in both conventional and integrative medicine, with decades of clinical data.

Why it matters

Chemotherapy is heavily metabolised by the liver and causes significant hepatotoxicity — elevated liver enzymes (ALT, AST, GGT) are among the most common reasons for dose modifications. Silymarin protects hepatocytes from oxidative damage, reduces ALT/AST elevations, supports liver cell regeneration, and has its own emerging anti-cancer properties.

The evidence

Multiple clinical trials support silymarin for hepatotoxicity from medications, alcohol, and mushroom poisoning. A 2010 published study found SilyPharm reduced liver enzyme elevations in paediatric ALL patients receiving chemotherapy. Intravenous silybin (Legalon SIL) is used in European intensive care for acute liver failure. Phytosomal formulations (Siliphos) have significantly better bioavailability.

Dosing

420–600mg/day standardised silymarin (70–80% silymarin content). Phytosomal form (Siliphos, Milk Thistle phytosome) achieves 4–10× better bioavailability than standard extract. Take with meals. Lagosa is a common pharmaceutical-grade brand available in Slovakia and Central Europe.

Cautions

Potential CYP3A4 interaction — space from chemotherapy administration timing and check with oncologist about timing relative to infusion days. Can mildly lower blood glucose. Very safe at standard doses with excellent long-term tolerability. Lagosa (brand) is available on prescription in some Central European countries.
