Resveratrol

Activates sirtuins, reduces inflammation, and mimics aspects of caloric restriction.

What it is

Resveratrol is a stilbene polyphenol found in grape skin, red wine, and Japanese knotweed (the primary commercial source). Trans-resveratrol is the active isomer. It activates SIRT1, a protein deacetylase central to longevity, DNA repair, and metabolic regulation.

Why it matters

Resveratrol activates sirtuins which regulate DNA repair, inflammatory signalling, and energy metabolism. It reduces NF-κB and COX-2 activity, inhibits cancer cell proliferation, and sensitises tumour cells to apoptosis. Its caloric-restriction-mimicking properties are of particular interest in the context of cancer metabolism.

The evidence

Over 100 clinical trials. Trans-resveratrol is the biologically active form — racemic mixtures are less effective. Bioavailability is inherently low but improved with pterostilbene (a methylated analogue), liposomal forms, or micronised preparations.

Dosing

100–500mg trans-resveratrol daily with food and fat. Combining with pterostilbene (50–100mg) enhances SIRT1 activation. Grape seed extract provides complementary polyphenols. Effects are cumulative — consistent use over weeks is necessary.

Cautions

May interact with blood thinners and CYP450-metabolised drugs. Estrogenic activity at high doses — discuss with oncologist if on protocols for hormone-sensitive cancers. Avoid high doses during pregnancy.
