Indirect Bilirubin (Unconjugated)

Elevated when red blood cell breakdown is the primary driver.

What it is

Indirect bilirubin is the fat-soluble, pre-hepatic form produced when haemoglobin is broken down. It is calculated as total minus direct bilirubin.

Why it matters

A selective rise in indirect bilirubin points toward haemolysis (red blood cell destruction), which can occur with certain treatments or autoimmune conditions. Identifying the pattern helps focus investigation.

How to test

Calculated from total and direct bilirubin in a standard liver panel.

Optimal range

Below 12 µmol/L normal.

How often

When total bilirubin is elevated.
